Please do not send these documents to our offices unless you are asked to do so.
- New host applications – please print and complete WWC Attachments 4 & 5 and have original or witnessed photocopies of ID ready for collection during your inspection. PLEASE DO NOT SEND THESE TO OUR OFFICES. You will not be offered a student until all permanent adult residents in your home have completed this.
- Annual Working with Children Screening renewals for registered hosts – please print and complete WWC Attachments 4 & 5 and forward to our office. You will be advised when this is required
- Applicant Declaration and Consent 2010 >>
- Proof of Identity 100 points form >>
- Guardian Carer Form 2009 form >>
for your information only, you will be sent a copy of this form if you host an underage student which you should complete and return - Homestay Information for Parents 2009 form >>
